All claims for health and dental services filed to the Department of Corrections for inmates must include an ODOC Inmate Number. This number must appear in box 1a on a CMS 1500, box 60 on a UB04 and box 15 on an ADA 2006 form. Zeros must be added to the number as a prefix when it is submitted on a claim form so that there are a total of nine digits in the inmate number. The claim will be denied if the inmate number is not present on the claim or if there are not nine digits in the number.
